Flown Apollo 12 Command Module instrument panel data card, dated October 20, 1969.

It lists details of the Service Propulsion System (SPS) burn, and the procedures for a surge tank leak and other eventualities. Signed on the back by Richard Gordon stating that the card was “utilized by me on the Apollo 12 mission.” 3½ x 6 inches. 3 patches of Velcro on verso to hold it on the panel.

Interesting that they used a mix of Celsius and Fahrenheit.
